    Intérêts de recherche

    Comme néonatologistes, nous faisons souvent face aux séquelles de la prématurité. Malgré que le taux de survie des prématurés ait augmenté depuis les deux dernières décennies, la morbidité surtout neurologique demeure élévée. Notre laboratoire étudie les mécanismes impliqués dans la pathogénèse, particulièrement de désordres neurodéveloppementaux (y compris la leucomalacie et l’hémorrhagie périventriculaire) ainsi que la rétinopathie du prématuré, la plus importante cause de cécité chez l'enfant. Compte tenu de l’importance de la microcirculation tant dans la genèse de ces pathologies (secondairement à sa dégénérescence) que dans la revascularisation tissulaire, l'accent est mis sur les mécanismes sous-jacents de ces processus. Nous nous concentrons sur les radicaux libres, les facteurs pro-inflammatoires ainsi que sur les nouveaux médiateurs de l’angiogénèse.

    Notre équipe a découvert les rôles importants de ces facteurs.

    • Nous avons découvert les effets cytotoxiques et les mécanismes sous-jacents de nouveaux produits de peroxydation et identifier pour la première fois leur récepteur.
    • Nous avons trouvé de nouveaux récepteurs couplés aux protéines G au niveau de la membrane nucléaire, ayant comme rôle spécifique la régulation génomique, ce qui diffère de leur rôle sur la membrane plasmatique, et décrit que les mécanismes de signalisation couplés à ces récepteurs suivent ces derniers dans le noyau cellulaire.
    • Nous avons découvert de nouveaux médiateurs agissant comme maîtres régulateurs de l’angiogénèse, ce qui constitue une des plus importantes trouvailles sur la néovascularisation;
    • Nous avons décrit une première dans le rôle des récepteurs vidangeurs, CD36, dans la génèse de la forme sèche de la dégénérescence maculaire liée à l’âge.
    • Nous avons trouvé de nouvelles plateformes dans le développement de modulateurs allostériques de récepteurs à structure complexe, tels que les récepteurs contenant des tyrosines kinases. Cette découverte a mené au développement de puissantes molécules innovatrices anti-inflammatoires de 4e génération.
    • Nous avons démontré les bénéfices de l’ibuprofène par rapport à l’indométacine dans la fermeture du canal artériel du nouveau-né prématuré. L’ibuprofène a été approuvé par la European Medicines Agency (2004) et la Food and Drug Administration (2006), et est présentement employé régulièrement en clinique.

    Il est donc clair que les concepts que nous avons développés n’ont pas seulement des conséquences sur le nouveau-né mais également sur d'autres pathologies telles les ischémies cérébrales de l'adulte, le diabète, l'athérosclérose et la dégénérescence maculaire liée à l’âge. De plus, nos recherches ont mené des applications cliniques concrètes. Notre équipe travaille présentement à mettre au point d’autres modalités thérapeutiques innovatrices.
